This waiver can be used for adults or minors.  There is another option for minors that is a more easily grasped, though, so we recommend it for minors. 

 

This waiver may be used for adults or minors, but it was created to be more suitable for minor’s understanding.  If you’re an adult, please use the “Universal Waiver.”  If you are a parent or guardian, please use this waiver.

 

This must be signed before playing. Don’t forget your waiver, as well.
